If you believe your record is incorrect or incomplete, you may request an amendment in writing. We may deny requests if we determine the existing record is accurate, but you will receive a written explanation for any denial.
Right to an Accounting of Disclosures
You may request a list of certain disclosures of your PHI made by our office, excluding those for treatment, payment, or operations. A reasonable fee may apply for repeated requests.
Right to Request Restrictions
You may request that we restrict how your information is used or shared. While we are not required to agree to all requests, we will honor any restriction that is feasible and does not conflict with legal obligations.
Right to Confidential Communications
You may request that we contact you at a specific address, phone number, or email for privacy reasons. We will accommodate reasonable requests.
Right to a Paper Copy
You have the right to obtain a paper copy of this notice at any time, even if you have reviewed or signed it electronically.
Right to File a Complaint
If you believe your privacy rights have been violated, you may file a complaint with our office or directly with the U.S. Department of Health and Human Services (HHS).
You will not be penalized or retaliated against for filing a complaint.

Office Policies
At Lavender & Lace Aesthetics and Wellness, we value your time and strive to provide every patient with the best possible care and experience. To ensure smooth scheduling and fairness to all clients, please review our office policies below.
Late Cancellations
We kindly request at least 24 hours’ notice for any appointment cancellations or reschedules.
Cancellations made within 24 hours of your appointment time are subject to a $50 late cancellation fee.
Missed Appointments (No-Shows)
If you fail to attend your appointment without prior notice, you will be charged the full cost of the scheduled service.
Repeated no-shows may result in restricted access to future bookings at our discretion.
Late Arrivals
If you arrive late, we will do our best to complete your service in the remaining time. However, to stay on schedule for other patients, your appointment may need to be shortened.
Please note that abbreviated appointments are still charged at full value.
Refunds
All sales are final.
We do not offer refunds on services rendered, or on any prescription or compounded medications, even if you are dissatisfied with the outcome. Our team will always work with you to address any concerns and help you achieve the best possible results.
